The CCA student insurance plan includes:

1. One Medical Membership (healthcare provider)
2. Health Insurance Coverage (through Aetna)
3. Dental and Vision Insurance Coverage (through Anthem Student Advantage)

One Medical Membership

Students enrolled in the student insurance plan are eligible for a One Medical Membership that will expand their healthcare network even further. One Medical provides high-quality, patient-centered primary care services with same-day/next-day appointments and has office locations in the Bay Area and nationwide.

One Medical accepts the CCA Aetna PPO plan as well as many other national insurers. Students also have access to 24/7/365 virtual care by phone, email, and video conference, a mobile app for appointment scheduling and prescription refills, personal health coaching, and referrals to a network of specialists.

Activating your Membership

• Visit onemedical.com/mybenefit, select the "Activate Now" button, and follow the instructions.

Health Insurance

As a reminder, you will need your Aetna Health card and ID number for any health care appointments.

To obtain your Health ID Card:

• Go to aetnastudenthealth.com/cca
• Select “Get your ID Card”
• Enter your school issued Student ID number and your Date of Birth in the field, then click “Submit"
• Select your ID card to display it on the screen
• Take a screenshot to save it to your phone or select Print your ID Card

• We encourage you to register here, but first you will need to get your Aetna ID number, which you can do by reviewing the option above.

Dental and Vision Insurance

Anthem Dental and Vision Coverage is included for each student that enrolls in the CCA Student Health Insurance Plan.

Students may purchase Anthem Dental and Vision Coverage for their dependents for an additional cost online through our student health insurance brokers at JCB (www.jcbins.com).

Accessing your dental and vision benefits

• Download the Sydney Health App on your Apple or Android device to get one-click access to benefits info, your member ID card, wellness resources, and find in-network providers.
• You may visit www.anthem.com or call Anthem directly for more information on your coverage or with questions regarding claims/bills.
